            MEANS ABILITY, MEANING, AND PURPOSE 

AMP provides residential services across Pennsylvania for youth and adults with intellectual and developmental disabilities (IDD), autism, trauma and mental health needs. Our residential services are built on person-centered care and guided by the "Everyday Lives" principle, as outlined by the Pennsylvania Office of Developmental Programs (ODP):

  1. We value what is important to people with disabilities and their families who are striving for an everyday life.
     

  2. People with disabilities have a right to an everyday life — a life that is no different than that of all other citizens.

 

At AMP, we build a sense of support and community around individuals while promoting health and safety. Each person supported by AMP lives a meaningful life as they define it. We match individuals with staff that are well trained and a good fit, identify meaningful activities both at home and in the community, and design short- and long-term goals with the necessary supports needed to reach them. We’re open and honest in our communication with families, service coordinators, and providers while providing exceptional care.

  • AMP's small, community-based homes across Pennsylvania are uniquely and personally designed for comfort and stability. Individuals are given a home to call their own and have agency on how their home looks and feels to meet their needs. AMP homes are accessible through environmental modifications. All safety modifications respect an individual's autonomy and human rights​.
     

  • AMP provides personal care, steady routines, and real relationships with familiar staff. We connect individuals to resources like Social Security, SNAP, financial management, and other resources and skills individuals may need. AMP staff assist individuals in planning trips and vacations.
     

  • AMP hosts sponsored events like our annual picnic for all individuals and their families, seasonal holiday celebrations, birthdays, weekly bingo, craft nights and more.

            RESIDENTIAL SERVICES AND ELIGIBILITY 

AMP Residential Rehabilitation and Supported Living Services:

  • Residential habilitation: is the most intensive of these options in which individuals stay in their own home provided by AMP, and transportation and 24/7 staffing support is provided. At AMP, we work with individuals and their support team to create customized goals to promote independence.

  • Supported Living Services: is a less intensive residential option. Supported living services can be provided in an individual’s own home or in a home provided by AMP. Supported Living provides up to 40 hours of staffing support weekly to assist the individual with living successfully in the community. This can include but is not limited to: assistance with scheduling appointments, medication management support, engagement with daily living activities and working toward goals contained within the Individual Support Plan (ISP).

AMP residential services follow four core foundations:

  1. Independent Living Skills: AMP supports independent living skills by creating SMART goals (Specific, Measurable, Attainable, Realistic within the Timeframe). Person-centered planning is then created. Progress is tracked and encouraged. Individuals supported work up to their abilities, and beyond, in a consistent manner that promotes their own independence. Individuals learn home skills, cooking, getting a valued job, taking courses or classes of interest, developing mutual relationships, friendships or romantic relationships, and community involvement that establishes pride in oneself and belonging.

  2. Community Involvement: AMP believes in the "Everyday Lives Principle," as defined by the Pennsylvania Office of Developmental Programs (ODP), where "people with disabilities have a right to an everyday life - a life of dignity, participation, and inclusion in the community." Every individual should be involved in their community to the extent that they choose. Together, our individuals identify ways to be involved in the community, putting together a plan of support that ensures health and safety. ​​​Community Involvement includes employment, volunteering, advocacy, memberships in organizations and clubs, and activities with peers and family members. AMP staff also assist individuals in forming healthy social connections, outside of paid staff, through community events, clubs and local organizations that are of interest to the individual.
     

  3. Medication Monitoring: Quality of life is paramount for every individual AMP supports. We train our staff according to regulations set by the Pennsylvania Office of Developmental Programs (ODP) and all staff are certified by the state of Pennsylvania. After certification, AMP are re-certified annually and monitored monthly. ​​​Individuals are given the agency to make informed decisions about their care. Individuals are trained by AMP staff to understand what medication is prescribed, how the medicine can help, and any possible side effects. Individuals can refuse treatment or medication. Individuals can self-administer their medication by being trained by AMP staff and their self-administer goal is then added to their support plan. AMP follows a 15-step monitoring process to pass and document medications. AMP uses documentation to track medication ordering, medication administration, to changes in treatment plans, and discontinuation of medication. AMP utilizes doctors, psychiatrists, behavioral specialists, therapists and pharmacists to help properly monitor and guide the medication prescribed. AMP staff educate individuals on how to advocate for themselves at appointments by assisting with accessible communication in understanding treatment plans and goals. AMP staff schedule, attend and document all doctor appointments and are up to date on individuals' diagnoses, prescribed medications and possible drug interactions. AMP Program Supervisors, Team Leads and certified Direct Support Professionals (DSPs) monitor medication multiple times a day by documenting medication counts, administration, and most importantly the effect the medication has on the individual. AMP Program Supervisors review medication records weekly and Team Leads review medication records daily. Within every 90 days, AMP staff monitor the effect of medication and ensure the diagnosis and prescribed medication is correct by consulting with the individual and their care team.
     

  4. Healthy Living: AMP listens to individuals' needs on what they find important to maintain a healthy and fulfilled life. Individuals partner with staff by creating grocery lists and taking the lead in grocery shopping, participating in favorite exercise activities together and providing actionable plans for maintaining a healthy lifestyle. Individuals learn the following Healthy Living skills: creating grocery lists and taking the lead in grocery shopping; preparing meals that contain the 4 food groups (milk, meat, fruit and vegetables, and grains); learning the importance of staying hydrated and having liquid available at all times; choosing exercise activities based on personal interests and skill levels; how to apply for memberships to gyms, indoor parks, indoor/outdoor pools, and how to join and participate on a sports team. AMP staff participate with the individual in swimming, sports, and other activities to ensure safety and community building

To be eligible, individuals must meet all four of the following criteria:

  1. An intellectual and developmental disability (IDD) diagnosis, autism diagnosis, and/or mental health diagnosis

  2. Receive service coordination services

  3. Be referred by a service coordinator to AMP

  4. Have a county consolidated waiver
